These activities include, but are not limited to, the generation of an invoice for submission to your medical aid (if applicable), for the issuing of prescriptions, medical certificates, referral letters, laboratory request forms, radiology request forms, and any other forms required for your medical treatment.
5.2 We require your contact information should an emergency arise or for the follow up of any medically related situations i.e. results of tests, response to treatment, etc..
6. UPDATES AND YOUR RIGHTS
6.1 If you would like to update your records, you must inform the practice either via email to info@fertilityhealth.co.za on in person, where you will make those amendments yourself.
6.2 You have the right to ask for a copy of any personal information, via email (info@fertilityhealth.co.za) or in person, that we hold about you in our records, to correct any inaccuracies and to update any out-of-date information. Please note that we are entitled to charge a small fee for providing you with copies of personal information we may hold about you.
7. CHANGES TO THIS PRIVACY POLICY
7.1 We reserve the right, at any time, to modify, alter, or update this Privacy Policy.
7.2 When we do so we will publish the changes on the Site. If you do not agree to these changes, please do not continue to use the Site.
